Financial Policy - We are happy to file with your insurance company when you provide us with the necessary information and referrals. Please be prepared to pay all charges at the time of service unless prior arrangements have been made with our financial counselor. Regardless of your insurance coverage, final financial responsibility rests with the patient. We recommend checking with your insurance company for specific requirements in order to obtain maximum benefits. Occasionally an insurance carrier will deny payment on a portion of a bill, which, in their judgement, "exceeds usual and customary." Please be aware that each carrier determines its own standard of "usual and customary", using their own statistics and regional influences. Our fees are based on the amount of time, work and skill required to provide the most appropriate care. The amount of this denial will be the responsibility of the patient. Privacy Practices - Carolina Hand and Sports Medicine, PA has a legal responsibility to protect the privacy of your personal health information (PHI). In addition, we are required by law to provide you with this notice, and to follow the information practices that are described therein. It is the policy of Carolina Hand and Sports Medicine, PA to follow all federal and state laws and reporting requirements regarding identity therft. Therefore, effective May 1, 2009, all patients will be required to bring the following to their next appointment: - Driver's license or other government issued photo ID (including passports) ;
- Current health insurance card;
- Utility bills or other correspondence (including paycheck stubs, etc.) showing current residence if the photo ID does not show the patient's current address. If the patient is a minor, the patient's parent or guardian should bring the information listed above.
